Objective -Subglottic infantile haemangioma (SGH) is a rare but potentially life-threatening disease. We describe a case of successful treatment with propranolol of a severely respiratory compromised 2-month-old infant with an obstructing SGH. Case report -A fullterm 2-month-old male infant, weighing 4.8 kg, without cutaneous haemangioma, presented with stridor, dyspnoea, oxygen desaturation and tachycardia. Contrast enhanced neck-chest CT scan was used to diagnose a 5×7×7 mm subglottic elliptic lesion, referable to SGH with a free air column of 2 mm wide. Per oral propranolol was started with a therapeutic dosage of 2 mg/kg/day in 3 administrations. On the second day of treatment there was rapid clinical improvement of the infant with withdrawal of respiratory symptoms. After treatment, the control CT showed the regression of SGH. Twenty-one months later the patient is without respiratory symptoms. Conclusion -Propranolol medical treatment should be considered in all subglottic haemangioma, even in acute cases.

Raktažodžiai: hemangioma, galva ir kaklas, konservatyvus gydymas, chirurginis gydymas, propranololis. SantraukaHemangioma -tai dažniausiai pasireiškiantis nepiktybinis navikas galvos ir kaklo srityje. Iki šiolei pirmo pasirinkimo vaistas gydant šios srities hemangiomas buvo peroraliniai kortikosteroidai, tačiau nauji tyrimai pagrindžia β blokatorių veiksmingumą. Straipsnyje aptariami šiuolaikiniai galvos ir kaklo hemangiomų konservatyvaus gydymo principai bei intervencinės procedūros. Pristatomi dviejų suaugusių moterų, kurioms diagnozuotos hemangiomos ryklės bei liežuvio šaknies srityse, klinikiniai atvejai. ĮvadasHemangioma (HA) -tai kraujagyslinis nepiktybinis navikas, dažniausiai pasireiškiantis antrą -dešimtą gyvenimo savaitę 2,5 proc. naujagimių [1]. HA atsiranda dėl per greito kraujagyslių išvešėjimo, kuris neatitinka audinio ir organo vaskuliarizacijos poreikio ir nėra pateisinamas fiziologiškai. Dažniau HA pastebimos moterims (moterų ir vyrų santykis=2:1) [2].Nors HA dažniausiai pasireiškia vaikams, jos gali atsirasti ir vyresniame amžiuje -paprastai vyresniems nei 40 metų pacientams [3]. HA, pasireiškiančios suaugusiems, yra mažesnės palyginus su vaikų, tačiau pasitaiko ir didelių, susitelkusių į grupeles HA atvejų. Suaugusiųjų HA atsiradimo priežastis nėra aiški -keletas teorijų teigia genetinę kilmę ar HA atsiradimą dėl nuolat dirginančių cheminių medžiagų [3].Iki šiolei nusistovėjusios ir vieningos HA klasifikacijos nėra. Klinikiniame darbe priimtiniausia yra histologinė -susiformavimo klasifikacija, pagal kurią HA skirstomos į keletą kategorijų: 1) pagal kraujagyslinę sandarą -į kapiliarines, kavernines, arteriovenines; 2) pagal atsiradimo laiką -į įgimtas ir infantilines (1 lentelė) [4][5][6][7].
