Propranolol Versus Metoprolol for Treatment of Electrical Storm in Patients With Implantable Cardioverter-Defibrillator

“…On the other hand, recurrent VT/VF (electrical storm) requires immediate and intensive antiarrhythmic management. The most commonly used drugs include amiodarone and β blockers (21,22). In patients refractory to pharmacological managements, percutaneous SG block and left cardiac sympathetic denervation might be effective (23-25).…”

“…In our study, we have found that treating electrical storm with the combination of propranolol (a nonselective β-blocker) leads to marked improvement of outcomes compared to β2 selective metoprolol (13). Could adrenergic effects on mitochondria account for that difference?…”

“…Since the ES is an emergency, it is initially managed with the advanced cardiac life support (ACLS) protocol [ 6 ], regardless of the etiology of the electrical storm. In advanced settings, the treatment is more etiology specific with pharmacologic choices of amiodarone, quinidine, propranolol, metoprolol, and isoproterenol or interventional therapies with ICD or ablations [ 7 11 ]. Neumar et al compared the combination of IV amiodarone and oral propranolol to the combination of IV amiodarone and oral metoprolol in the management of ES in ICD patients and found amiodarone and propranolol to be superior, safe, and effective in ES management [ 6 ].…”
