Prospective Study of Neuropathic Symptoms Preceding Clinically Diagnosed Diabetic Polyneuropathy: ADDITION-Denmark

Abstract: To evaluate whether diabetic polyneuropathy (DPN) follows the hypothesis for the course of nerve fiber damage reflected by symptoms progressing from pure small through mixed to large nerve fiber symptoms with or without symptoms of loss of function of small nerve fibers. RESEARCH DESIGN AND METHODSRepeated assessments of nerve fiber-specific symptoms were obtained in 518 participants of the ADDITION-Denmark study from the time of a screeningbased diagnosis of type 2 diabetes using specific items of the Michiga… Show more

“…In this study, our cohort was age‐ and sex‐matched across T2D, T2D DPN, and lean groups. The diabetes groups exhibited the anticipated metabolic characteristics, for example, elevated HbA1c, glucose, BMI, waist circumference, which were observed in similar cohorts 7,27,28 . Basic plasma lipid characteristics in the diabetes group revealed lower total cholesterol compared to lean controls, presumably due to statin use.…”

“…Kalın liflerin tutulumu, uyuşukluğa ve duyu kaybına neden olur (12). Duyu kaybı, distal sensorimotor polinöropatinin varlığını gösterir ve diyabetik ayak gelişimi için bir risk faktörüdür (13,14). Diyabet süresi ve glisemik kontrolün derecesi insidans ve ciddiyeti etkileyen majör faktörlerdir (15).…”

“…pain or paraesthesias may be related to the degree of compensatory regeneration rather than to the degree of nerve fiber damage. Moreover, symptoms may have a heterogeneous long-term course with progression and regression to a similar extent 49 . Screening tools for neuropathic pain may offer guidance for further diagnostic evaluation and pain management but do not replace clinical judgment.…”
