Evaluation of the etiology of ocular globe atrophy or loss

This survey investigated the etiology of atrophy or loss of the ocular globe in patients assisted at the Maxillofacial Prosthetics Clinic of two Schools of Dentistry in São Paulo State, Brazil. A total of 238 patients were examined and their clinical files were reviewed. The etiology of eyeball atrophy/loss was assessed with respect to gender, age group, affected side and type ophthalmologic surgery performed. The greatest incidence of ocular globe loss was due to traumatic etiology (57.14%), followed by pathogenic (36.13%) and congenital (5.04%) etiologies. Comparing the genders, a predominance of male patients was observed (61.76%; p<0.01). The age group most frequently affected was between 21 and 40 years (42.01%; p<0.01). For all types of etiologies investigated in this study, enucleation was the most commonly used surgical procedure for removal of the ocular globe (66.38%; p<0.01). Loss of the left eye was predominantly seen (55.04%), even though no statistically significant difference was found between sides (p>0.01).

O presente trabalho visou relatar um caso clínico de deformidade facial de etiologia traumática, reabilitado através de prótese óculo-palpebral no Programa de Extensão em Prótese Bucomaxilofacial da Faculdade de Odontologia da UFRGS. Assim como levantar informações que sustentem uma discussão sobre esse tema, permitindo um amplo e detalhado conhecimento. Descrição do caso: Paciente A.O.N., 62 anos, sexo masculino, relatou acidente automobilístico há 40 anos e ter realizado 10 cirurgias plásticas até o momento para corrigir a deformidade. Em um primeiro momento foi realizada a moldagem da região óculo-palpebral e confeccionado modelo de trabalho em gesso. Após foi confeccionada a prótese ocular com a íris obtida pela técnica da pintura a mão. No modelo de gesso com a prótese ocular em posição foi realizada escultura com plastilina. Com a escultura pronta foi realizada a inclusão da peça em mufla e a inserção do silicone na cópia negativa com a realização da pintura intrínseca. Com o paciente presente, foi realizada a pintura extrínseca e caracterização com cílios. A prótese foi fixada com cola, e foi recomendada a utilização de óculos para apoio da prótese. Conclusão: Nem sempre é possível restaurar os defeitos da face com uma abordagem cirúrgica, desta forma o conhecimento por parte do cirurgião-dentista sobre as reabilitações protéticas faciais é importante, pois as mutilações faciais provocam alterações morfofuncionais e psicossociais nos pacientes portadores das deformidades, levando-os ao isolamento social e familiar. Através das reconstituições protéticas faciais, o indivíduo poderá ser reintegrado à sociedade e melhorar sua qualidade de vida.

“…The absence of ocular content may be congenital [1] or acquired due to surgical removal which can be indicated in several cases such as traumas [2][3][4], cancer, blind painful eye, microphathalmos, endophthalmitis, and suprachoroidal hemorrhage [3]. The ocular defects can be corrected by prostheses which have many functions such as restore the esthetic, prevent eyelid deformation, protect the anophthalmic cavity, orientate the lacrimal flux, and avoid its accumulation in this cavity [5]. Furthermore, the ocular prosthetic rehabilitation is associated to psychosocial improvement, once the prostheses are able to influence positively the interpersonal relations [6,7], which leads to a positive impact on the quality of life.…”
