Proton pump inhibitors as a risk factor for paediatric Clostridium difficile infection

Abstract: SUMMARY BackgroundProton pump inhibitors (PPIs) and H 2 receptor antagonists (H 2 RAs) may play an important role on the onset of Clostridium difficile-associated disease (CDAD) in adults. The impact of Clostridium difficile on children treated with gastric acid-suppressing agents remains unknown.

“…To evaluate the role of potential risk factors for CA-CDI, comorbidity data were evaluated, including pre-term birth (gestational age <37 weeks), previous gastrointestinal surgery, solid organ and stem cell transplantation, malignancies, chemotherapy, use of proton pump inhibitors, immunodeficiency, renal failure (peritoneal dialysis or haemodialysis), cystic fibrosis, inflammatory bowel disease (IBD) or Hirschsprung's disease, prior hospitalization, G-or Jtube feedings, and exposure to oral or parenteral antibiotics within the 4 previous weeks [28,29].…”

“…We did not do a proper analysis of the use of proton pump inhibitors, which has shown an association with CD in other paediatric studies. 36 Our study contributes interesting information to the clinical profile of children with ICD and other causes of diarrhoea. Infection by other pathogens was typically found in older children and associated with more severe symptoms, often presenting with fever, haematochezia, increased frequency of bowel movements and laboratory abnormalities.…”

“…Al igual que en esta investigación, diversos estudios (3,4,16,22,23) han documentado que esta enfermedad afecta a todos los grupos poblacionales sin distinción de edad ni de sexo, aunque parece presentarse con mayor frecuencia en los adultos mayores de 65 años, en quienes, además, es más agresiva (3,12,22,24).…”
