Pruritic urticarial papules and plaques of pregnancy in twin and triplet pregnancies

Abstract: The reported rate of PUPPP in single pregnancies is 1 in 200 (0.5%). Our findings suggest that multiple pregnancies are at a higher risk of developing this cutaneous eruption with 2.9% of twin and 14% of triplet pregnancies affected in our study.

“…The second most common dermatosis observed in our patients' series, PEP (21.6%), occurred particularly frequently in primigravidae (73%) and multiple gestation pregnancies (16%), as previously reported. 11,12,[24][25][26] PEP started in late pregnancy (83%) or the immediate postpartum period (15%), with conspicuous abdominal involvement, seen in 98% of cases, characteristically affecting the striae distensae and sparing the periumbilical region. In contrast, PG, the only other dermatosis with marked abdominal involvement (95%), favored the periumbilical region in 72%.…”

“…The higher rates of PUPPP reported in multiple gestation pregnancies [10], maternal excessive weight gain, and nulliparity [1] have led to several hypotheses regarding the etiology of this condition. Stretching and breakage of skin connective tissue can produce an immune reaction by revealing antigens otherwise hidden from the maternal immune system [11].…”

“…It is seen most commonly in first pregnancies and with multiple births [233]. It presents typically with erythematous papules within the striae and these spread to extremities but spare the face, palms, and soles.…”
