Pseudo-leptospires in blood culture.

Abstract: SUMMARY Spiral filaments seen in the blood cultures of two patients with fever and jaundice were initially thought to be leptospires; these were later proved to be artefacts. An investigation was carried out to exclude the possibility of laboratory contamination of the culture media and to find out how these objects were produced. The significance of the findings is discussed in relation to the possibility of a mistaken diagnosis in routine laboratories which have a limited experience of leptospires. Two patien… Show more

“…Although there have been several reports of pseudospirochaetes, most of them (Noguchi, 1926;Rahman & Macis, 1979;Frerichs & Maley, 1980;Sparling & Baseman, 1980;Faine, 1982) describe 'pseudoleptospires' (spiral elements like leptospires), which have been frequently encountered in blood cultures and mistaken for true leptospires. The difficulty in differentiating leptospires from pseudoleptospires has been stressed.…”

“…6,8,[11][12][13][14][15] These reports often suggest that these artifacts occur as a result of an in vitro event, such as heat treatment. In this report, the in vitro event appeared to be incubation at 35 C in BSK medium.…”

“…The use of broad spectrum antibiotics and correct isolation media (Palmer et al 1984) are factors that determine the rate of isolation. Furthermore, the detection of leptospires by dark-field microscopy is limited, requiring the presence of viable motile bacteria that may be confused by the presence of proteinaceous filaments or pseudo-leptospires (Rahman & Macis 1979).…”
