Pseudomyxoma peritonei treated by cytoreductive surgery and hyperthermic intraperitoneal chemotherapy: results from a single centre

Abstract: Cytoreduction plus peritonectomy procedures combined with HIPEC is a safe treatment and could improve survival rates. Since the optimal cytoreduction is the primary prognostic factor, patients should be centralised under the care of experienced teams.

“…Histological subtype is widely deemed a major prognostic factor for long‐term survival in PMP patients, as indicated by Ronnet et al [25]; a number of authors associate DPAM with a better prognosis than the carcinoma‐related subtypes (PMCA‐I and PMCA) [6, 7, 9, 15, 17–19] reaching up to 100 % in 5‐year survival in some cohorts [10, 18, 19]. The rates of survival are very different in the reported cohorts with a very similar treatment approach using the maximal cytoreduction and HIPEC.…”

“…All patients in the study underwent surgery at least once at our institution during the course of disease treatment. The principles of cytoreductive surgery and HIPEC as performed at this center and by this team have been reported previously [10, 12]. Briefly, cytoreductive surgery consisted of resecting all visible tumor deposits, depending on disease spreading; surgery also may include resection or electrofulguration of implants on mesenteric visceral surfaces and small bowel, with excision of affected organs.…”

“…It has historically been considered a terminal condition for which debulking surgery or palliative treatments have been the only therapeutic options available. In recent years, several peer-reviewed studies have published results showing a survival benefit when treating PMP with cytoreductive surgery (CRS) combined with hyperthermic intraperitoneal chemotherapy (HIPEC) [1][2][3][4]. However, despite these therapeutic efforts, recurrence is common, with subsequent progression and death occurring in the absence of effective treatment.…”

“…This issue of Clinical and Translational Oncology includes the experience in the management of PMP from one of GECOP's national referral centres for peritoneal surface malignancies [8]. Arjona-Sánchez and colleagues contribute in this way to raise awareness of the need for centralised, specialised management and add to the reported phase II efforts on this disease.…”
