This article considers some of the effects of health sector reform on human resources for health (HRH) in developing countries and countries in transition by examining the effect of fiscal reform and the introduction of decentralisation and market mechanisms to the health sector.Fiscal reform results in pressure to measure the staff outputs of the health sector. Financial decentralisation often leads to hospitals becoming "corporatised" institutions, operating with business principles but remaining in the public sector. The introduction of market mechanisms often involves the formation of an internal market within the health sector and market testing of different functions with the private sector. This has immediate implications for the employment of health workers in the public sector, because the public sector may reduce its workforce if services are purchased from other sectors or may introduce more short-term and temporary employment contracts.Decentralisation of budgets and administrative functions can affect the health sector, often in negative ways, by reducing resources available and confusing lines of accountability for health workers. Governance and regulation of health care, when delivered by both public and private providers, require new systems of regulation.The increase in private sector provision has led health workers to move to the private sector. For those remaining in the public sector, there are often worsening working conditions, a lack of employment security and dismantling of collective bargaining agreements.Human resource development is gradually being recognised as crucial to future reforms and the formulation of health policy. New information systems at local and regional level will be needed to collect data on human resources. New employment arrangements, strengthening organisational culture, training and continuing education will also be needed.

O governo eletrônico tem destaque na literatura como um importante instrumento de transparência da Administração Pública no Estado democrático. No entanto, várias são as dificuldades encontradas para alcançar os resultados almejados pelos governos a partir dessa estratégia, consolidando aspectos internos, como a estrutura do governo, e externos, como a participação dos cidadãos. Esta pesquisa propõe-se a fazer um mapeamento analítico da estrutura do governo eletrônico do Estado do Paraná, no que se refere à prática da transparência governamental. Para isso, utilizou o modelo teórico proposto por Prado (2009), constituído por três grupos de categorias que classificam gradativamente o desenvolvimento do governo eletrônico. Verificou-se que, apesar dos avanços conseguidos pelo governo eletrônico paranaense, este se encontra em estágios intermediários de desenvolvimento nas categorias de análise utilizadas para o quesito transparência.Governo Eletrônico. Transparência Governamental. Reforma da Administração Pública. E-government has been highlighted in the literature as an important Public Administration transparency tool within a democratic State. However, there are several difficulties to achieve the results desired by governments using that strategy, covering internal aspects, such as the governance structure, and external ones, such as citizens' participation. This research proposes to make an analytical mapping of the structure of e-government in the state of Paraná, regarding government transparency. To do so, we used the theoretical model proposed by Prado (2009), which consists of three groups of categories that gradually classify the development of e-government. Tais reformas buscam a flexibilização da estrutura burocrática com o objetivo de alcançar melhor eficiência de gestão pública e de atingir melhores desempenhos no que se refere à prestação dos serviços públicos. Os estudos sobre as organizações públicas em um processo democrático vêm apresentando essa mudança de ênfase da Administração Pública sob uma dicotomia entre política e administração, ou seja, uma separação entre os papeis do exercício político tal qual é apresentado pela Ciência Política, de sua função executiva com foco na eficiência, como apresentada pelos administradores. Para Denhardt (2012), apesar de acreditar que essa dicotomia ainda esteja muito presente nas práticas atuais da Administração Pública, seria adequado a promoção de uma interação entre política e administração. Fiscal transparency and the cost of sovereign debt

This article analyses the factors that seem to play an important role in determining the cost of sovereign debt. Specifically, we evaluate to what extent transparency, the level of corruption, citizens' trust in politicians and credit ratings affect interest rates. For that purpose, we create a transparency index matching the 2007 Organisation for Economic Co-operation and Development/World Bank Budgeting Database items with the Organisation for Economic Co-operation and Development Best Practices for Budget Transparency sections. We also check our assumptions with the International Budget Partnership's Open Budget Index and with a non-linear transformation of our index. Furthermore, we use several control variables for a sample of 103 countries in the year 2008. Our results show that better fiscal transparency, political trust and credit ratings are connected with a lower cost of sovereign debt. Finally, as expected, higher corruption, budget deficits, current account deficits and unemployment make sovereign interest rates increase. Points for practitionersThe key implications for professionals working in public management and administration are twofold. First, despite the criticism raised by credit ratings, it is clear that poorer ratings are connected with higher financing costs for governments. Therefore, governments should enhance those indicators that impact the credit rating of their sovereign debt. Second, governments should seek to be more transparent, since transparency reduces uncertainty about the degree of cheating, improves decision-making and therefore decreases the cost of debt.
