Pulmonary Coccidioides nodule in a Swiss patient with chronic lymphatic leukaemia

Abstract: The dimorphic fungus Coccidioides immitis, causative agent of coccidioidomycosis, is endemic in semi-arid regions of the Americas. The disease is rarely encountered in Europe. We describe the case of a 61-year-old Swiss patient with chronic lymphatic leukaemia who was eventually diagnosed with an initially unnoticed pulmonary coccidioidomycosis.

“…Infection with Coccidioides immitis , a soil-dwelling fungus endemic to the American Southwest, may have protean manifestations in immunocompromised patients who reside in1 2 or who have travelled to3 that region. We report a patient whose abnormal PET/CT scan raised concern for recurrent lymphoma but instead was due to disseminated coccidioidomycosis.…”

“…However, at a time when the movement of people and their companion animals is increasingly common, it is important to be aware that they can carry a variety of infectious agents from endemic to disease‐free regions, thus contributing to the emergence of imported diseases. In fact, during the past decade, some cases of coccidioidomycosis have been reported in European and Asian citizens who had travelled to endemic areas (Chandesris and others 2008, Hombach and others 2008, Indhirajanti and others 2009, Kwok and others 2009). However, to date there are no documented cases of coccidioidal infection in animals outside endemic regions.…”

“…Recently, some cases of coccidioidomycosis in human beings were reported in Europe and Asia; in each case there was a history of travel to endemic regions (Chandesris and others 2008, Hombach and others 2008, Indhirajanti and others 2009, Kwok and others 2009). However, to the best of the authors' knowledge, no cases of coccidioidal infection in animals from outside endemic areas have been previously reported.…”

“…Surgical drainage of fluid collections in skin, joint or soft tissue and systemic treatment are the cornerstones of therapy. (84)(85)(86)(87)(88) Reactivation of endemic fungal infections such as histoplasmosis, blastomycosis and coccidiomycosis can occur in the setting of immune suppression and can occur several years after leaving the region of original infection. Histoplasmosis may present in a disseminated fashion.…”
