Pyogenic granuloma associated with Actinomyces israelii

Abstract: Peculiar findings of orofacial actinomycosis mimicking the clinical appearance of a tumor of the upper gingiva are reported. An 83-year-old man with bleeding of the gingiva visited our hospital. The clinical diagnosis was a benign gingival tumor, and the lesion was surgically removed. Histologically, the excised specimens showed an ulcerative granuloma lesion covered by bacterial colonies consisting of club-shaped filaments. DNA samples were extracted from paraffin sections and examined by polymerase chain rea… Show more

“… 6 In oral soft tissues, lesions associated with Actinomyces of the gingiva, the tongue, the buccal, and the tonsil have been reported. 7 , 8 , 9 In this study, four of the cases (44.4%) were suspected to have tumors at initial diagnosis. Additionally, various antibiotics were used based on different initial diagnoses.…”

“…For example, a gingival inflammation, like the periodontal abscess and bone loss, is a fertile ground at local irritants, which permits the ingress into the gingival connective tissue of low virulent oral microflora [11]. In this frame, the result of some studies shows that a group of around 14-15 species of Actinomyces and especially Actinomyces Israeli, which is detected normally in oral flora, incriminates for increase of probability of PG [12]. Moreover, PG case reported in patient with Sturge-Weber Syndrome (SWS), or Encephalotrigeminal angiomatosis.…”

“…Beside brucellosis, TB is not the only infectious disease to develop granulomas. Other intracellular bacteria known to develop this kind of structures are Coxiella burnetti (etiologic agent of the Q-fever) [ 206 ], Bartonella henselae (Cat-scratch disease) [ 207 ], Tropheryma whipplei (Whipple’s Disease) [ 208 ], Actinomyces israelii (actinomycosis) [ 209 ], and Salmonella enterica Typhimurium (gastroenteritis) [ 210 ], among others.…”
