Quality of ultrasound biometry obtained by local health workers in a refugee camp on the Thai–Burmese border

Abstract: ObjectiveIn a refugee camp on the Thai–Burmese border, accurate dating of pregnancy relies on ultrasound measurements obtained by locally trained health workers. The aim of this study was to substantiate the accuracy of fetal biometry measurements performed by locally trained health workers by comparing derived reference equations with those published for Asian and European hospitals.MethodsThis prospective observational study included 1090 women who had a dating crown–rump length (CRL) scan and one study-appo… Show more

“…This corroborates findings from a similar comparison conducted in Thailand. 40 Mean FLs observed in urban PNG were higher than our findings after 30 GWs. This may because their cohort contained a higher percentage of Papuans/Polynesians than our cohort, but it could also be because they used biparietal diameters for dating and performed cross-sectional analysis of a longitudinal dataset.…”

“…Of the 441 scans performed during the study period, 214 were reviewed with 96% agreement with fetal heart rate, 91% agreement with placental location, and 70% agreement with BPD. Overall, 2 articles by Rijken et al9, 10 evaluated the accuracy of fetal biometry performed by locally trained health workers in a refugee camp on the Thai-Burmese border. For biometry between 16 and 40 weeks GA, the health workers, trained in a 3-month course, showed an intraclass correlation of >0.99 in the first study 9 and low-standard deviation compared with values derived from European and Asian studies in the second study 10 .…”

“…Infants who are born before 28 weeks are given supportive care and excluded from this analysis, as described and recommended by Turner et al [21]. The estimation of gestation with ultrasound before 14 weeks of pregnancy is reliable and the quality of ultrasound service in the SMRU clinics is high [26]. If the mother attends ANC after the 24 weeks of gestational age the next best estimate will be chosen from the Dubowitz test [27] or last menstrual period if known.…”
