Quantification of Fatty Degeneration Within the Supraspinatus Muscle by Using a 2-Point Dixon Method on 3-T MRI

Abstract: An increase in supraspinatus fatty degeneration was statistically significantly correlated with severity of supraspinatus tears and moderately correlated with muscular atrophy. Fatty degeneration tends to progress more rapidly than muscular atrophy in female patients, with statistically significant sex differences.

“…Consequently, several simplified versions or quantitative (i.e. objective) adaptations of Goutallier grading system have successively been proposed [8,15,16,[18][19][20][21]. Of particular interest is the study by van de Sande et al, who described producible CT method to quantify fatty infiltration of rotator cuff muscles.…”

“…However, the parasagittal reconstruction plane was adjusted manually, thereby representing a source of error, and the authors only measured the mean muscle density rather than quantifying muscle atrophy and fatty infiltration separately [21]. Further quantitative methods using MRI fat-water separation techniques have recently been reported [18,19,22]. Fat fraction values in rotator cuff muscles were found to be reliable and correlated well with shoulder pain and range of motion [22], the severity of SS tendon tears [18], and the occupational ratios [19].…”

“…Despite attempts to standardize the grading system, there is lack of quantitative data in the classification of Goutallier et al Recent efforts to more precisely quantify the amount of fatty infiltration have shown improved reliability with measurement of Hounsfield units on CT images [19] and T2-sequence mapping [23] or two-point Dixon sequence measurements [26] for MRI scans. In addition, the relatively wide range of five stages has been cited as a potential reason for low reliability [32] and has resulted in simplified three-stage classifications.…”
