Quantification of Neighborhood-Level Social Determinants of Health in the Continental United States

Abstract: IMPORTANCE An association between social and neighborhood characteristics and health outcomes has been reported but remains poorly understood owing to complex multidimensional factors that vary across geographic space. OBJECTIVES To quantify social determinants of health (SDOH) as multiple dimensions across the continental United States (the 48 contiguous states and the District of Columbia) at a small-area resolution and to examine the association of SDOH with premature mortality within Chicago, Illinois. DES… Show more

“…Covariate variables associated with comorbid conditions, included education level, household income, and age. From the historical record and previous studies, we know that discrimination plays a role in educational attainment and income levels, which alludes to the greater impact of discrimination on not just health, but other factors that influence health including social determinants of health (Knapp & Hall, 2018;Kolak et al, 2020). Importantly though, even when controlling for these other known social determinants of health, participants' race remained a significant predictor.…”

“…For minority racial/ethnic populations, stress arising from discrimination may play a role in terms of the physiological response to chronic stress, which has been linked to poor health outcomes among minority populations (Carter et al, 2017;Gee et al, 2007;Williams 2018). Additionally, discrimination can have community level impact by influencing availability of housing options, food insecurity, and access to health services, all of which are shown to be associated with poor health outcomes (Knapp & Hall, 2018;Kolak et al, 2020).…”

“…Another example is the use of neighborhood deprivation metrics that can assess health outcomes and disparities within regions (for example, affluent towns may have pockets of health deprivation) [ 23 ]. Another example is a recent study [ 24 ] of census tracts across the US that classified social determinants of health measures using indices of advantage, isolation, opportunity, and migrant cohesion and accessibility. The analysis was conducted by 7 neighborhood typologies, which included an extreme poverty group.…”

“…Social determinants of health are associated with violence. 18 Violent crime data were collected from Federal Bureau of Investigation Uniform Crime Reports Offenses Known to Law Enforcement Search Engine. 19 Violent crime data included rates per 100 000 of homicide, rape, robbery, aggravated assault and total violent crime.…”
