Radiation diagnostics of arteriovenous erectile dysfunction: history and development

Purpose: to evaluate the possibilities of CT cavernosography in the preoperative preparation of patients, in order to determine the size of cavernous bodies, for the correct selection of the size of pemile implants. Material and methods: 26 patients who underwent penile prosthesis implantation with preoperative preparation in the form of CT cavernosography, in order to determine the size of cavernous bodies. Results: In 15 patients, the data on the sizes of cavernosography coincided with the true ones without errors, in 11 patients the error was 0.5 cm (which was compensated by the nozzles from the kit). During the observation of complications after the installation of penile prosthesis implantation associated with improper selection of size was not fixed. Conclusion: CT cavernosography is an effective way of preoperative planning of patients with the aim of clarifying the size of cavernous bodies and selecting the right size of the prosthesis. This method allows you to select an implant with an accuracy of 0.5 cm, which in turn will reduce the risk of complications associated with improper selection of prostheses, as well as improve the economic component in the form of reducing the number of cases when the prosthesis was opened with an incorrectly selected size, which queue leads to financial loss of the clinic.

The third millennium marked an increase in interest in the quality of human life and well-being. Human sexual health has become an integral part of a harmonious life. Great interest in the problem of erectile dysfunction is due to the increase in the number of patients with this pathology in the world. Many scientific studies have made progress in understanding the physio logy of erection. Starting its development from an objective examination, questioning, assessment of psychological history, the diagnosis of erectile dysfunction has stepped to new heights, such as multislice dynamic pharmacocavernosography using 3D modeling and positron emission tomography in combination with visual sexual stimulation or erectogenic pharmacological stimuli. The article presents data on the historical aspects of the study of erectile dysfunction and trends in the development of its diagnosis from ancient times to the present day. The main instrumental research methods that are actively used to diagnose the disease at the present time are described.

A review of modern methods of diagnosis and treatment of vasculogenic erectile dysfunction is presented. Data regarding the diagnostic value of noninvasive and invasive diagnostic techniques are displayed: stamp test, Snap-Gauge test, use of the RigiScan and Androscan-MIT systems, intracavernous injections of vasoactive drugs, Doppler ultrasound of penile vessels, penile angiography, and magnetic resonance arteriography. The advantages and disadvantages of each of these tests are shown. The main directions of conservative and surgical treatment of vasculogenic erectile dysfunction are presented. Notably, early detection of erectile dysfunction with the prescription of appropriate treatment can improve the quality of life of patients and prevent life-threatening complications of cardiovascular conditions.
