Radiographic Diagnosis of Tarsal Coalition

Abstract: Routine anteroposterior and lateral unenhanced radiographs are a valuable screening tool for tarsal coalition, even when used by inexperienced observers. The newly described signs increase sensitivity of radiographic diagnosis.

“…The tarsal coalition decreases the normal motion of the hindfoot, and the talonavicular joint develops compensatory increased motion; consequently, the superior aspect of the articular facet of the talar head shows a focal bony prominence related to the formation of a traction osteophyte at the talar insertion of the dorsal talonavicular ligament. The sensitivity and specificity of the talar beak sign in detecting talocalcaneal coalition are 48% and 91%, respectively, thus making it a helpful sign if present, but the absence of the talar beak cannot be used as a helpful distinguisher [5]. It is important to note that there are several other osseous outgrowths that occur along the dorsal aspect of the talus that are not associated with tarsal coalition.…”

“…It must be stressed that the radiograph needs to be obtained with the patient positioned properly, as improper positioning can either create a false sense of the sign being present or a false negative absence of the sign. The sensitivity has been reported as high as 100%, which makes absence of the sign a reliable way to exclude talocalcaneal coalition [5]. However, the specificity of the sign is only 42% making its presence rather nonspecific [5].…”
