The levels of IgG1, IgG2, IgG3, and IgG4 were analysed by ELISA in cord serum and in serum samples collected at 6 and 18 months of age from infants whose mothers were atopic. None of the four IgG subclasses was significantly influenced on any sampling occasion by infant atopy, gender, month of birth, maternal IgE or maternal diet during pregnancy and early lactation. However, at 18 months of age, significantly higher levels of IgG1 (P less than 0.05) and of IgG4 (P less than 0.01) were found in infants with an elevated IgE (greater than or equal to 8.0 kU/l) than in those with a lower level. A weak positive correlation (rs = 0.26; P = 0.05) between IgE and IgG4 was also observed. Despite the fact that the serum levels of IgG4 at 18 months were significantly higher (P less than 0.01) among infants with positive IgE-RAST (greater than or equal to 0.15 PRU/ml) to ovomucoid or beta-lactoglobulin, our data suggest that the the concentration of IgG4 relates more to the level of IgE than to the clinical symptoms of atopy. Determination of IgG subclasses seems to be of limited value for prediciting atopy during early infancy.

Summary:The antibody isotype response to Trichinella spiralis excretory/secretory (ES) products of muscle larva was examined using sera from patients with confirmed trichinellosis. Using Western blots we identify components of the ES antigen that are recognized by IgM and IgG antibodies. A 45 kDa component was strongly recognized by different antibody classes and subclasses. We observed a 45 kDa-specific lgG4 response that was detected exclusively using sera of patients with trichinellosis and not of patients with echinococcosis, filariasis, cysticercosis, ascariasis, strongyloidiasis or toxocariasis. These results are relevant for the diagnosis of human trichinellosis.KEYWORDS : human trichinellosis, ES antigen, 45 kDa, lgG4.M onitoring programs for detection of trichinellosis in slaughterhouse animals have dramatically decreased the incidence of human trichinellosis. However, in many parts of the world, including Europe, it continues to occur. Recent outbreaks of trichinellosis have been reported from Germany, France, Spain and Italy (Ancelle et al, 1998;Rodriguez et al., 1999); (Pozio 1998).In our laboratory the immunodiagnosis of trichinellosis is performed by using an enzyme-linked immunosorbent assay (ELISA) in which the excretory/secretory (ES) products of Trichinella spiralis muscle larvae are used as antigen (van Knapen et al., 1982 MATERIALS AND METHODST he excretory/secretory (ES) antigen was prepared using viable muscle larva of T. spiralis as previously described by Gamble (1985). The 45 kDa antigenic component was purified from freshly obtained muscle larva using two-step affinity chromatography as described by Homan et al. (1992) SERUM SAMPLES Serum samples from trichinellosis patients recently received at our laboratory and from an outbreak of trichinellosis that took place in Slupsk, Poland in 1991 were used in this study. The patients here studied were all at an early stage of trichinellosis. All serum samples used tested positive for IgM and/or IgG by means of ELISA using T. spiralis ES antigen. Infection was confirmed for several patients from the outbreak in Poland and for the other patients used in this study by identification of Trichinella larva in muscle biopsy.The source for T. spiralis infection for the outbreak was undercooked pork. Sera from our laboratory from echinococcosis, filariasis, cysticercosis, ascariasis, strongyloidiasis or toxocariasis patients were also included. Various studies have suggested the involvement of immunoglobulin G4 (IgG4) antibodies (Ab) in the physiopathology of allergic disorders. Recently, an abnormal IgG4 Ab production in response to immunization has been reported in some atopic patients. Thus, in order to evidence in allergic patients, a potential abnormal IgG4 Ab response to aeroallergens following natural exposure, we compared, in 34 patients sensitive to Dermatophagoides pteronyssinus and in 16 healthy subjects, the IgG4 Ab response to D. pteronyssinus, grass pollen and cat dander, using a solid-phase radioimmunoassay. Since some patients were also sensitive to grass pollen and/or to cat dander, we analyzed, in all patients, the IgG4 Ab responses both towards the allergen(s) they were sensitive to (sensitizing allergen) or not (unrelated allergen). The results showed that 90% of the patients produced levels of antisensitizing allergen(s) IgG4 Ab significantly higher than the controls; this IgG4 Ab response was correlated with the corresponding specific IgE Ab level. In addition, among these patients, around 40% presented high levels of IgG4 Ab to the unrelated allergen(s). Thus, in allergic patients, while specific IgE Ab define the nature of the sensitizing allergen, the presence of IgG4 Ab directed against various allergens seems in relation with an abnormal isotype regulation associated with atopic disorders.
