Randomized Controlled Trial of Daily Total End Range Time (TERT) for Capener Splinting of the Stiff Proximal Interphalangeal Joint

Abstract: Further research with a larger sample is needed to determine whether longer daily TERT is beneficial. Our results suggest, however, that most patients find it difficult to wear splints >12 hr/day.

“…Presumably, increasing the time in the cast was felt to increase the dose of treatment received. This practice conflicts with the findings of Kolumban 24 and Glasgow et al 25 that indicate that there is no significant difference in the effectiveness of passive PIP joint extension casts/splints if used for 12 versus 24 hours per day.…”

“…This allows the child to interact with the environment during the day. Total amount of time to wear the orthosis is 6e9 h. 11,12 There is poor corroboration of outcomes for conservative management of camptodactaly in the scientific literature and limited demonstration of long eterm benefit from conservative management. While there is evidence of short-term advantage, the conservative management demands are labourous for the patient and family, and difficult to ensure consistent compliance over time.…”

“…A TERT wear of 10.8 hours (SD: 2.1) was reported. Glasgow, Fleming, Tooth, and Peters (2012b) randomly allocated daily TERT wear of a dynamic capener orthotic regimen as either 6-12 or 12-16 hours. A daily mean TERT of 9.5 hours for the first group and 11.5 hours for the 12-16 hours group was reported (Glasgow et al, 2012b).…”
