Objective Streptococcus pyogenes also called Group A streptococci (GAS) is an important pathogen known since past years and infects millions of people around the world every year and can cause death. Although it often causes pharyngitis and skin infections, it can also spread to the bloodstream and cause invasive GAS (iGAS) infections. In this review, studies on the epidemiology of GAS and iGAS and vaccine studies were reviewed. Methods A comprehensive search of databases was conducted up to April 1, 2023 to related studies, in English using the following terms: S. pyogenes, GAS, invasive S. pyogenes infections, iGAS, S. pyogenes vaccine. The data extraction was conducted by two investigators using the same methodology. Results There has been an increase in iGAS infections in various countries in the last year and they have been followed closely. Although the reason for the increase in iGAS cases cannot be determined exactly, it is thought that the recent removal of coronavirus disease 2019 restrictions and the increase in the number of individuals susceptible to GAS and to viral respiratory pathogens is thought to have caused this situation. Another important issue with GAS is the vaccine studies. Vaccines are based on two different targets: M protein-based and non-M protein-based. Phase-I clinical trials have been initiated for some vaccines. Conclusion The increase in recent iGAS infections will draw attention to this agent and accelerate vaccine studies.

Point-of-care (POC) devices play an important role in the protection of public health by providing rapid diagnosis of infectious diseases, patient management, and effective treatment. Fast, easy-to-interpret, environmentally resistant, and cost-effective POC tests that can be used practically in the field are gaining more and more importance every day. There is a need for portable devices that will enable rapid diagnosis kits to be used in the field for early diagnosis and treatment. The aim of this study is to evaluate the DNA hunter device that was developed in terms of providing the required temperature for M. tuberculosis (MTB) diagnosis of the loop-mediated isothermal amplification (LAMP) assay and visually evaluating the analysis results. The device in this study; handheld (total weight 430 g, outer dimensions 70 x 175 x 80 mm), the average operating time can reach a maximum temperature of 110 degrees in 2 minutes with a fully charged battery, and the processing time is about 90 minutes without being connected to electricity. It can display the pre-evaluation result on the screen with the full digital color sensor. The device can be adjusted to the desired reaction temperature and time. It also has software where sample registration numbers can be entered. DNA Hunter can be used for all analyses performed by the LAMP method and the results can be evaluated colorimetrically, thus it is well suited for POC testing.

Pharyngitis is an inflammatory condition of the pharynx and/or tonsils commonly seen in both children and adults. Viruses and bacteria represent the most common encountered etiologic agents—yeast/fungi and parasites are infrequently implicated. Some of these are predominantly observed in unique populations (eg, immunocompromised or unvaccinated individuals). This manuscript (part 2 of 3) summarizes the current state of laboratory and point-of-care diagnostic testing and highlights the expanding role of nucleic acid amplification in the expedited diagnosis and management of patients with acute pharyngitis. It discusses preanalytical, analytical, and postanalytical variables that impact the performance of culture, rapid antigen, and nucleic acid amplification testing. Finally, it sets the stage for part 3, which discusses the emerging role of biomarkers in the management of individuals with acute pharyngitis.
