Re-expansion pulmonary oedema after minimally invasive cardiac surgery with right mini-thoracotomy

Abstract: RPO should be recognized as one of the most serious complications after MICS with right mini-thoracotomy. More accurate risk factors of prolonged lung malperfusion and steroid use on RPO after MICS should be investigated.

“…The use of dexamethasone during cardiopulmonary bypass had a protective effect (28), which lends support to the possibility of an inflammatory trigger. However, unilateral pulmonary edema after minimally invasive surgery is rarely clinically apparent (28,30). In our experience, this problem is rare (in our subjective perception/observation <1%).…”

“…What is striking, however, is that all publications on this subject used one-lung ventilation through a double-lumen endotracheal tube. The studies described the following risk factors: • Pulmonary hypertension • Administration of fresh frozen plasma (28)(29)(30).…”

“…The vast majority of previously described of RPE have occurred after lung inflation in patients with persistent compression of pulmonary parenchyma, but a few reports of RPE after lung deflation of relatively short duration also have appeared [1]. There have been few reports of RPE that developed after cardiac operations, but many of these resulted from operations through a right thoracotomy approach [2,3]. Although a number of cases appear to occur with valve operations, the duration of pulmonary collapse is shorter considering the lung area.…”
