Reaction of facial soft tissues to treatment with a Herbst appliance

Abstract: The influence of the Herbst appliance on the facial soft tissues is expected to result in a positive treatment outcome, particularly in patients with a convex profile, a retrusive lower lip, and a marked sublabial fold. We observed a broad clinical spectrum of individual reactions in the facial soft tissues. It is, thus, not possible to detect a linear relationship between the Herbst treatment and soft tissue changes, making soft tissue changes difficult to predict.

“…[6][7][8] The appliance also generates advancement of the soft tissue of the chin and of the lower lip, contributing to improvements of the facial profile esthetic. 3,9,10 Despite its clinical efficiency, the Herbst appliance has some disadvantages, such as the need for complex laboratory preparation, operator experience, and frequent repairs as well as initial soft tissue discomfort experienced by the patient. 11 Currently, the Forsus device is an interesting alternative, since it has also been shown to be relatively efficient in Class II correction of mild-tomoderate cases.…”

“…5,7,10,11 Most of the studies using 3-D facial imaging technologies in orthodontics have dealt with the changes achieved by orthognathic surgery 6,8,9 and various types of orthodontic treatment. 12,13 However, there have been only a few 3-D photographic studies on soft tissue changes around the lips by orthodontic appliances 5 after debonding. Therefore, the purpose of this study was to evaluate the changes in soft tissues around the lips after removing brackets using 3-D light stereophotogrammetry.…”
