Recognition and management of abdominal compartment syndrome among German pediatric intensivists: results of a national survey

Abstract: IntroductionSeveral decades ago, the beneficial effects of goal-directed therapy, which include decompressive laparotomy (DL) and open abdomen procedures in cases of intra-abdominal hypertension (IAH) in children, were proven in the context of closures of abdominal wall defects and large-for-size organ transplantations. Different neonatologic and pediatric disease patterns are also known to be capable of increasing intra-abdominal pressure (IAP). Nevertheless, a considerable knowledge transfer regarding such r… Show more

“…There is little agreement on the indications for open abdomen treatment and what type of temporary abdominal closure devices should be used [ 14 18 ]. Most recent studies conclude that awareness among health care providers improved over recent years, but guidelines are still not uniformly applied or knowledge was inadequate [ 19 – 21 ].…”

“…9 Abdominal compartment syndrome, or IAH, may be diagnosed through a combination of clinical indicators, as well as serial measurements. 11 Once ACS has been diagnosed, the intra-abdominal pressure must be promptly relieved to avoid complications. 10 This entity may or may not have a rapid onset and may develop over several days; thus, an ongoing high clinical suspicion is warranted.…”

“…ACS is not rare and has a high mortality in the paediatric intensive care unit; the incidence and mortality rate are 0.6%–4.7% and 16%–86%, respectively 2. Although physicians should recognise and intervene rapidly in paediatric ACS cases, previous reports revealed that awareness of it was not sufficient 3 4. There are few reports about recognition and intervention for paediatric ACS in the emergency department (ED).…”
