IntroductionCompare cost of the interstitial liquid glucose flash monitoring (FM) system (FreeStyle Libre 2) versus self-monitoring of blood glucose (SMBG) in adults with type 1 diabetes mellitus (T1DM) in Spain.Research design and methodsA model was developed to estimate, with the perspective of the Spanish health system, the annual costs associated with glucose monitoring and hypoglycemic events management in T1DM population, with multiple insulin daily doses (MDI). According to published evidence, rate of severe hypoglycemia (SHE) of 4.90 episodes per patient-year was applied. Reduction of SHE (58.6%) was modeled associated with FM use. Published rates of hospital care (20.2%) and subsequent admission (16%) were assumed for SHE. The daily consumption of strips and lancets was 9 in patients with SMBG (before and after 4 daily intakes and at bedtime) and 0.5 for FM users (according to IMPACT trial findings). Annual consumption of 26 FM sensors was considered (1 every 14 days). Unit costs (in € of 2019, excluding VAT) were obtained from literature and national databases. Sensitivity analyses (SA) were carried out to evaluate the model robustness.ResultsThe total annual cost/patient was €4437 for SMBG and €2526 for FM. The use of FM would be associated with an annual savings in the costs of monitoring and managing hypoglycemic events of €1911 per patient-year. In a hypothetical cohort of 1000 patients with T1DM MDI, FM could avoid in 1 year 4900 SHE, 93 hospitalizations for SHE. In addition, the use of FM would generate total savings of up to €1 910 000 per year. In the SA with alternative hypoglycemia events rates and use of strips and lancets, and including non-SHE episodes, savings from €370 000 to €1 760 000 were observed with FM.ConclusionsThe use of the FM system to monitor glucose in adults with T1DM treated with MDI, would reduce hypoglycemic events and would result in cost savings for the health system.

Objetivo Evaluar el cumplimiento normativo del servicio de glucometría en droguerías del departamento de Santander-Colombia. Métodos Estudio observacional descriptivo de corte transversal. Se usó un instrumento de evaluación basado en la norma y validado por expertos. Resultados Se aplicó una encuesta a 68 droguerías. Los resultados fueron categorizados así: recursos humanos, dotación, infraestructura, documentación-registro y procesos de vigilancia y control. En la primera categoría se identificaron falencias relacionadas con la formación académica del personal a cargo de la droguería, con un porcentaje de cumplimiento del 66,2% y en la capacitación del personal en el manejo del equipo de glucometría 35,3%. En general el conocimiento de la normatividad por parte del personal fue de 36,8%. En las categorías de dotación e infraestructura se evidencia un nivel de cumplimiento superior al 50% para todos los ítems, excepto el relacionado con la calibración del equipo. En la cuarta categoría, específicamente endocumentación y registro, los niveles de cumplimiento normativo son inferiores al 50% en todos los ítems, excepto el de información, que se le ofrece al paciente durante el procedimiento. Los resultados evidencian una falla en los procesos de vigilancia y control, dado que solo el 57,4% de los establecimientos confirman visitas de inspección. Conclusiones Los resultados de este estudio revelan un desconocimiento generalizado de la normatividad en droguerías que prestan el servicio de glucometría; consecuentemente, no se cumplen todas las exigencias regulatorias. Fomentar programas de capacitación y fortalecer la cultura de la autoevaluación garantiza procesos seguros para el paciente.

Objective To assess concordance between two point‐of‐care testing (POCT) devices and the standard laboratory method in screening for gestational diabetes mellitus (GDM) in Huesca. Methods Pregnant women who met criteria for an oral glucose tolerance test (OGTT) and attended the laboratory between October 2017 and November 2018 were recruited in this prospective observational study. Glucose was measured in venous (laboratory) and capillary blood (Accu‐Chek or Contour Next glucometers). GDM was diagnosed attending to NDDG criteria for venous samples or capillary‐specific cut‐off. Linear regression, Passing–Bablok, Bland–Altman, and the kappa coefficient were used to study concordance between POCT and laboratory method. Results Data from 109 women were analyzed (57 for Accu‐Chek, 52 for Contour Next). Statistical analyses showed good agreement between both POCT and laboratory method. There were no statistical differences in fasting glucose measurements between capillary and venous samples and both POCT devices meet the ISO 15197 standard. Accu‐Chek showed good agreement (k=0.629) regarding the laboratory method in classifying GDM, with an acceptable inter‐evaluator bias of 3.5% (P<0.001). Conclusion POCT can be used to obtain fasting values and reduce overall waiting times for patients. Additionally, Accu‐Chek can be used to diagnose GDM in remote areas applying specific cut‐off values.
