Reconstruction of the midface and maxilla

Abstract: Multiple reconstructive pathways can be followed in restoring maxillary and midface defects. The surgeon and rest of the reconstructive team must make individualized decisions based on the nature of the defect as well as the specific needs and concerns of the patient when selecting the optimal reconstructive approach for each patient.

“…Traditionally, midfacial defects are obturated with bulky dental prostheses. At present, the free flap has become the preferred method for reconstruction of midfacial defects in adults as well as in children . Few studies have compared the rates of flap failure for the midface and lower face.…”

“…The two contrasting views are whether a bone-containing flap or myocutaneous flap with a free bone graft is needed for maxilla reconstruction. 28,29) Surgeons who advocate bone-containing flaps frequently used the scapula-latissimus osteomyocutaneous flap, including the thoracodorsal arterial system to reconstruct the orbital floor and/or anterior arch. 30) This flap is considered applicable to various defects of the maxilla, because each of the reconstruction materials can be used flexibly.…”

“…In the case of partial and total maxillectomy, effective reconstructive surgical options are well outlined in the literature . Brown type 3 and type 4 resections usually mandate some form of immediate reconstruction, whereas Brown type 1 and type 2 resections usually are amenable to prosthetic rehabilitation or surgical reconstruction.…”
