Recurrence of Dupuytren’s contracture: A consensus-based definition

Abstract: PurposeOne of the major determinants of Dupyutren disease (DD) treatment efficacy is recurrence of the contracture. Unfortunately, lack of agreement in the literature on what constitutes recurrence makes it nearly impossible to compare the multiple treatments alternatives available today. The aim of this study is to bring an unbiased pool of experts to agree upon what would be considered a recurrence of DD after treatment; and from that consensus establish a much-needed definition for DD recurrence.MethodsTo r… Show more

“…However, this information is valuable to patients and policymakers, as such serious complications are likely to have a more major effect on quality of life, and lead to increased healthcare costs. Similarly, our definition of re-operation on the same hand is not identical to the recently agreed consensus for a definition of recurrence 22 . It is possible that some re-operations were for extension of disease to another finger, and that patients with true recurrence decided to not have repeat surgery.…”

“…Since these studies can provide useful information for health care providers and enhance health and therapeutic interventions to improve the quality of services. Ultimately these could lead to improving the quality of life of the patients [ 81 83 ].…”

“…Furthermore, the literature lacks a clear universal definition of disease recurrence and what constitutes a clinically relevant difference in total extension deficit, although an expert group recently reached a consensus that recurrence should be defined as more than 20° of contracture recurrence in any treated joint at 1 year post-treatment (or later) compared with 6 weeks post-treatment. 38 This in turn may affect the sample size calculation. We base our calculation on a clinically relevant difference of 20° since it has been used most frequently in recent studies of collagenase treatment 18 19 and is supported by the expert group.…”

“…We base our calculation on a clinically relevant difference of 20° since it has been used most frequently in recent studies of collagenase treatment 18 19 and is supported by the expert group. 38 Another limitation is the use of PROMs in research on DD. Although the QuickDASH score, a measure of activity limitations related to upper-extremity disorders, is commonly used in hand surgery research, it may not be sensitive to changes in DD-related contractures.…”
