Recurrent Ingrown Toenails Secondary to Indinavir/Ritonavir Combination Therapy

Abstract: With the increasing popularity of IDV/RTV combination therapy, clinicians should be aware of the potential increase in frequency of dose-related toxicities including IGTN. Evaluation of hands and feet on physical examination should be recommended for all patients being treated with lamivudine and IDV, especially when used in combination with RTV.

“…10), periungual fibroma in a lateral position, but also (amelanotic) melanoma and other malignant tumors (56) may cause ingrowing of the nail. Ingrown nails and granulation tissue mimicking them have also been observed after successful treatment of onychomycoses (57,58), under anti‐HIV therapy (59,60), and after oral retinoids (61). A thorough history is therefore mandatory, even in the seemingly banal case of ingrown nail.…”

“…[14] Ingrown toenail and paronychia have been reported secondary to drugs, such as indinavir and indinavir/ritonavir combination. [15,16] Excess nail fold granulation tissue and ingrown toenail have also been reported with retinoids, [17] docetaxel, [18] cyclosporine [19] and oral antifungal treatment. [20] Subungual neoplasms may cause ingrown toenails due to compression of the nail plate against the nail fold.…”

“…86,87 Retinoid-like effects are well established in indinavir-treated patients, including alopecia, chelitis, xerosis, and paronychia. [88][89][90][91][92] Garcia-Silvia et al 90 estimated that almost one-third of patients treated with indinavir developed at least two of these retinoid-like effects. 90 Indinavir has been shown to stimulate naturally occurring all trans-retinoic acid, and has been suggested to increase retinoid signaling pathways by decreasing the metabolism of retinoic acid.…”
