Recurrent Synovial Chondromatosis of the Index Finger Case Report and Literature Review

Abstract: We report a case of synovial chondromatosis of the proximal interphalangeal joint of the dominant right index finger with a follow-up of four years. The lesion recurred within a year of complete excision of the fibrous sac containing cartilaginous nodules. Despite early recurrence and extensive calcification around the proximal phalanx, no malignant changes were noted on histological examination. The patient remains asymptomatic for two years after the second operation.

“…Males are affected more than females, commonly ranging in age from the third to fifth decade of life [3,12,13,18]. Typically large joints are involved with the knee being the most common, followed by the hip and elbow [3,8,15,16,18,19,26].…”

“…Typically large joints are involved with the knee being the most common, followed by the hip and elbow [3,8,15,16,18,19,26]. This disease has been documented in the joints of the hand, specifically the metacarpophalangeal joint [2,7,11,20,22,23,26] and interphalangeal joint [6,10,[12][13][14]25]. Only two of these cases involved the thumb interphalangeal joint [13,14].…”

“…2,3 There have been 7 previously reported cases of synovial chondromatosis involving an interphalangeal joint. [3][4][5][6][7][8][9] Among these cases, recurrence was reported in only 2 patients. One case recurred 4 years after the primary excision of the lesion.…”

“…5 The second case recurred 1 year after the removal of cartilaginous nodules without synovectomy. 4 We describe a case of recurrent synovial chondromatosis of the distal interphalangeal (DIP) joint at the site of a previous removal of what was thought to be a ganglion cyst in 2003 and a subsequent recurrent synovial chondromatosis excision in 2005, both procedures done at an outside institution. The patient presented to us with local joint invasion and erosion, necessitating wide excision, bone grafting, and arthrodesis for definitive treatment.…”
