One of the key aspects that reveals the essence of the uneven distribution of social benefits in economically developed countries is the dynamics of spatial inequality that has been characteristic of the US healthcare sector for many decades. The results of an empirical study of this inequality as a dynamic process with the properties of historicism, carried out by decomposition of the Theil index according to the intragroup and intergroup characteristics of the corresponding groups of indicators, indicate an increase in the polarization of territories in terms of the provision of the population with medical benefits. Given the organizational and economic inconsistency in the health care of American citizens and the predominance of the spontaneous and commercial nature of free pricing in the relevant markets of American medical goods and services, the center-peripheral spatial structure of inequality in the development of US health care, traditional for capitalist reproduction, is confirmed.The objective nature of the long-term formation of such a structure is complemented by the subjective factor of state regional regulation of health care in the subregional and transregional sections, through the implementation of differently directed typologically relevant policies – from traditional and proactive to selective. It is shown that not only the economic situation, but also the socio-political and ideological expediency are the subjects that determine the tools for activating, polarizing, stimulating or restraining the development of healthcare in specific areas of the United States.

The Dynamics of Regional Economic Growth and the Priorities of US Regional Economic Policy

Stimulation of the pace and formation of the structure of regional economic growth is invariably one of the fundamental goals of the regional economic policy pursued by the subjects of the federal state. In the context of the cyclical development of regional economic systems and the beginning of the regionalization of the economy of the world leader – the United States of America - the problem of spatial and temporal differentiation of the choice of priorities to stimulate regional economic growth is becoming more urgent. Using the methods of regression modeling and economic statistics, an analysis was made of the influence of the main instruments of regional economic policy on the dynamics of regional economic growth over a long period of time 1961–2020 and forecast up to 2030. The obtained results revealed spatial asymmetries in the combination of the use of fiscal instruments and structural investment measures to regulate economic growth, associated with the level of development of subregions and individual states of the country. The transfer of the investment burden to stimulate regional economic growth in times of crisis to the end consumer is noted. The article substantiates the increasing role of such an instrument of regional policy as the selective regulation of aggregate regional export in the long term, indicating the transition from globalization to regionalization of the development of the economic systems of the states and largest municipalities of the United States to maintain the pace of regional economic growth while changing its structure.

Modeling the Spatial Development of the US Cross-Border Regions as Mesolevel Socio-Economic Systems

The cross-border nature of the spatial development of economically and socially significant regions identified in the geospace of the largest federal states of the modern world necessitates their study from the standpoint of the mesoeconomics of development as complex, open and dynamic systems in space-time. The object of this study is the cross-border regions of the United States with Mexico and Canada, which are distinguished by a characteristic set of specific properties of both system-structural and socio-economic nature. According to the systemic economic theory, the identified cross-border regions of the United States show the principle of duality of spatial development, and within the framework of evolutionary theory – a specific “socio-economic genotype” of the territory. Using elements of mesoeconomic modeling, the author identified and substantiated some trends in the spatial development of different groups of cross-border regions of the United States over a long time period of 1970–2020 and as a forecast until 2030. Based on a comparative analysis carried out on the results of modeling and testing, the author's hypothesis about a multidirectional change in the dynamics of indicators of socio-economic spatial development, which depends on the regional features of the spatiotemporal evolution of specific transboundary territories, is confirmed – as socio-economic systems of the mesolevel. The obtained results, revealing the spatial dependence of the data, indicate the growth of spatial relationships of both social and economic nature within the studied cross-border regions of the United States, which determine the spatiotemporal evolution of the latter in the geoeconomic and geocultural space of the United States and neighboring states. Based on the results of mesoeconomic modeling, the existence of an evolutionary-cyclical nature and duality of the studied mesosystems is empirically confirmed, having their own socio-economic genotype of territories, depending on the insurmountable specifics of spatial diversity and unevenness in the conditions of the market organization of American society. The main conclusion is that regional interest, which has dialectical unity and variability, manifested over time, causes different directions and levels of spatial development of specific regions of the United States as integral dynamic and open socio-economic systems of the mesoterritorial level, reflecting the gradual increase in economic regionalization. The approach used, taking into account the specifics of spatial development, is of interest for the study of similar subject issues in relation to large federal states that have a transboundary nature of genetically determined socio-economic interaction.
