Relationship between circulating sex steroid hormone concentrations and incidence of total knee and hip arthroplasty due to osteoarthritis in men

Abstract: Low plasma androstenedione concentration is associated with an increased risk of both knee and hip arthroplasty for OA for overweight and obese men. While the findings need to be confirmed in other cohort studies, they suggest that circulating sex steroids may play a role in the pathogenesis of OA in men.

“…After menopause, women have increased osteoarthritis risk. However, only a few studies have investigated the association between testosterone deficiency and osteoarthritis risk in elderly men, and those results have been inconsistent [ 1 , 2 ]. However, serum testosterone concentrations are positively associated with cartilage volume, but the loss of cartilage volume for 2 years does not induce osteoarthritis [ 2 ].…”

“…However, serum testosterone concentrations are positively associated with cartilage volume, but the loss of cartilage volume for 2 years does not induce osteoarthritis [ 2 ]. In a prospective cohort study, osteoarthritis risk is associated with lower serum androstenedione concentrations in obese and overweight men [ 1 ]. Therefore, lowering circulating serum concentrations of sex hormones may influence osteoarthritis etiology in men.…”

“…In animals, research suggests that Androstenedione has been linked to increasing bone mineral density (BMD) at metaphyseal and diaphyseal femoral regions as well as bone mineral quality and quantity of cancellous and cortical bone. The findings, however, do not clarify whether or not such effects come from a direct action of Androstenedione itself or the involvement of aromatase in the conversion of the steroid precursor to T, E2 or Ag within bone tissue [9], [13], [18].…”

“…Osteoarthritis (OA) is a major degenerative joint disease which can affect more than one quarter of the global population in individuals over the age of 18. This disease is typically defined by the following observations: progressive loss of articular cartilage, thickening of the subchondral bone, and formation of osteophyte, significant inflammation of the synovial as well as degeneration of ligaments and menisci of the knee and hypertrophy of the joint capsule.…”

“…persists for testosterone and estrogen between males and females, but notable differences have also been described for other hormones, including leptin [16][17][18][19] . The role that estrogen plays in OA has been well defined, as decreased expression of this hormone directly correlates to enhanced OA incidence [20][21][22] .…”
