Relevancia del antecedente de absceso perirrenal en pacientes sometidos a nefrectomía simple retroperitoneal

“…Even though in this study the presence of an abscess is taken as medical background, we can see how the presence of an abscess is an important factor for presenting perioperative complications, like the ones we describe. In this same study, one of the variables that was evaluated was blood transfusion between both groups, finding that blood transfusion was more frequent in those patients who had a history of abscess compared to those patients who did not have a history of abscess, obtaining even more relevance the results found in our study, however despite this difference it was not statistically significant in the article presented by Carrillo-Córdova et al 8 It is evident how the presence of the abscess in the context of stone disease is a complex pathology to be treated surgically that there are even studies where it is recommended that, given this finding, laparoscopic nephrectomy is not recommended due to the high conversion rate for a poor exposure to the renal hilum as well as bleeding. 9 Although we are not comparing minimally invasive versus open approaches, we can observe that the presence of an abscess is a factor that confers a significant risk for perioperative complications.…”

“…Una vez que se instaure la sospecha clínica, debe completarse el abordaje diagnóstico con tomografía contrastada con el objetivo de evaluar la presencia del absceso, características, así como planear el drenaje o tratamiento quirúrgico del absceso, debe iniciarse de inmediato el tratamiento con antibioticoterapia, ya que su progresión representa alto riesgo de morbimortalidad, con aumento del riesgo de avance de infección con afección de la función renal, posibilidad de choque, etcétera. 1,4,5 Se trata de una patología infrecuente como ya ha sido mencionado anteriormente, presentamos este caso ya que consideramos de interés y relevancia como aporte a la literatura médica y epidemiología actual por su peculiaridad, ya que la paciente no contaba con factores de riesgo para desarrollar un absceso renal, mucho menos complicaciones dada la particularidad en la que progresó.
