Remission of Antiepiligrin (Laminin-5) Cicatricial Pemphigoid After Excision of Gastric Carcinoma

Abstract: The present case differed from past cases by lacking inferior conjunctival sac shortening and by showing erosive lesions solely at the mucocutaneous junctions. The ocular involvement in this case correlated very well with the severity of gastric carcinoma.

“…16 There is one report of a patient with a B-cell lymphoma who actually exhibited combined immunofluorescent features of cicatricial pemphigoid and pemphigus vulgaris. 17 An association between mucous membrane pemphigoid and malignancies is supported by the fact that some reports have shown a parallel progression of the 2 diseases, where improvement of the mucous membrane pemphigoid has been accompanied by improvement in the cancer, and vice versa, 9,10,[12][13][14][15] The patient reported in this case showed a parallel improvement in his pemphigoid and malignancy following administration of cyclophosphamide. There is a reported association between glaucoma and MMP, 18 and it is interesting to note that this patient's intraocular pressures normalized after treatment of his MMP.…”

“…Previously reported cases have shown a temporal association between cicatricial pemphigoid and systemic solid malignancies, including non-small cell lung cancer (adenocarcinoma, squamous cell, large cell), gastric adenocarcinoma and endometrial adenocarcinoma 28. In some patients, the pemphigoid responded to chemotherapy or surgical resection of the tumour, suggesting a paraneoplastic process 2 6 8.…”

“…In some patients, the pemphigoid responded to chemotherapy or surgical resection of the tumour, suggesting a paraneoplastic process 2 6 8. Antibodies to laminin-5, a basement membrane protein elaborated by lung and gastrointestinal epithelia, have been found in patients with cicatricial pemphigoid and lung and gastric cancers 24 6 8. Aberrant synthesis of laminin-5 by tumour cells, or tumour-related inflammation and invasion, may expose the antigen to the immune system 2 9.…”

“…In some BP cases, tumor resection led to complete recovery [36] supporting the thesis of close interrelation of the diseases. Gastrointestinal tumors have been reported in association with BP [17,37], PNP [38], pemphigus vegetans [39], PV [40][41][42][43][44][45] and MMP [46] sometimes with post-excisional remission [47,48]. Both chronic leukemia and lymphoma have been reported with DH [49][50][51] and PNP [52][53][54].…”
