Renal Involvement in IgG4-Related Disease: From Sunlight to Twilight

Abstract: IgG4-Related Disease (IgG4-RD) is a fibroinflammatory condition characterized by a typical histopathological pattern (dense lymphoplasmacytic infiltrate with prevalent IgG4+ plasma cells and storiform fibrosis), which may involve the kidney both directly (IgG4-related kidney disease, IgG4-RKD) or indirectly, as a consequence of post-renal ureteral obstruction due to retroperitoneal fibrosis (IgG4-RD RF). The most frequent presentation of IgG4-RKD is IgG4-related tubulointerstitial nephritis (TIN), but a glomer… Show more

“… 1 The location of the urinary tract obstructions and the lesions causing it vary widely. 12 In addition, renal insufficiency can be caused by renal parenchymal lesions referred to as IgG4-related kidney disease (IgG4-RKD) 13 , 14 especially in IgG4-related RPF. Such renal involvement in RPF can lead to not only acute kidney injury (AKI) but also chronic kidney disease (CKD) including renal atrophy or even end-stage renal disease (ESRD).…”

“…Nevertheless, treatment regimens that combine glucocorticoids with cyclophosphamide or RTX have shown favorable outcomes. [17,18,43] In the present case, glucocorticoid treatment was initially effective for IgG4-RD when the patient presented with clinical symptoms. After two years of follow-up, the patient exhibited nephrotic syndrome and was diagnosed with PLA2R-positive pMN, characterized by positive PLA2R antigen expression and IgG1-dominant deposition in the glomerulus.…”

“…No fue posible efectuar la inmunohistoquímica por la escasez del material. La paciente realizó tratamiento con altas dosis de corticoides y posteriormente con rituximab, con evidente mejoría clínica en controles posteriores 1,[7][8] .…”
