Renal manifestations of sarcoidosis: from accurate diagnosis to specific treatment

Abstract: Sarcoidosis is a multisystem granulomatous disease characterized by epithelioid noncaseating granulomas associated with clinical and radiologic fi ndings. The cause of this disease is still uncertain. Sarcoidosis affects mostly lungs and lymph nodes and is not usually considered a urological disease, therefore, this etiology may be overlooked in several urological disorders, such as hypercalcemia, hypercalciuria and nephrolithiasis. It affects all races and genders. This review aims to describe the urological … Show more

“…Hypercalcemia in sarcoidosis is secondary to an uncontrolled synthesis of 1,25-dihydroxy vitamin D3 by macrophages present in granulomas. The increased 1,25 dihydroxyvitamin D3 leads to increased intestinal absorption of calcium that leads to increased resorption of calcium in the bone ( 139 , 140 ). The most common manifestation of abnormal calcium metabolism is hypercalciuria which is prevalent in ~40–62% ( 119 ).…”

“…Hypercalcemia indicating a possible kidney involvement ( 26 ) or infrequently bone marrow sarcoidosis ( 27 ), was present in 6% of the tested patients. However, none of the patients in our cohort suffered from renal or bone marrow sarcoidosis.…”
