Renal metastasis from papillary carcinoma thyroid detected by whole body iodine scan: A case report and review of the literature

Abstract: Papillary carcinoma is the most common thyroid malignancy. Usual sites of metastasis include lungs and bone, but renal metastasis is very rare. Here we present a case of a follicular variant of papillary carcinoma with renal and lung metastasis at presentation.

“…The prevalence in clinical series is lower and varies between 0.47% 87 and 5.3%. 6 Multiple case reports have been published in the literature 26,[88][89][90][91][92][93][94][95][96][97][98][99] and in general, metastatic renal tumors from thyroid primaries are multiple, bilateral, and usually accompanied by metastatic disease to other sites. 85 Breast Breast carcinoma represents another potential source for renal metastases, with a prevalence estimated between 4% and 14%.…”

“…The particular design of [ 89 Zr]Zr-DFO-Gal3-F(ab') 2 , a protein formed of two F(ab') fragments, results in faster blood clearance and lower liver uptake than traditional mAb-based tracers [125]. The high uptake of [ 89 Zr]Zr-DFO-Gal3-F(ab') 2 in kidneys is due to the urinary excretion [125], which should not be problematic because metastatic TC to the kidney is very rare [126]. For a diagnostic purpose, the dose is usually quite low and so is the nephrotoxicity.…”

“…Although DTC metastases to kidneys are very rare, some cases were indeed reported. On autopsy, 2.5 to 2.7% of all metastases to the kidney originated from thyroid carcinoma [12]. Some of them may be seen on WBS as a result of NIS expression [13].…”
