Resolution of peripheral artery catheter-induced ischemic injury in infants -Two case reports-

Abstract: The occurrence of catheter-induced vasospasm of small caliber arteries is a well known complication of arterial catheterization in neonates. However, there is rare publication about these complications in infants. We report infants with peripheral ischemia caused by arterial catheterization of the femoral artery.

“…Regarding papaverine, I5 is hesitant to its usage due to lack of experience as it is currently not used in obstetrical or neonatal Dutch clinics. Researchers working on peripheral arterial lines stated that papaverine could be effective to reduce vasospasm, but it should be used with caution in premature infants because of increased intracranial hemorrhages risks [ 37 ]. In all lamb experiments papaverine was given intravenous or topical [ 7 , 25 , 36 ].…”

“…No other medicaments were used to oppose vasospasm, such as lidocaine or nitroglycerine. Evidence of lidocaine’s effect on vasospasm is contradictory and may therefore not be used in lamb experiments [ 37 ]. In addition, nitroglycerine was discussed as vasodilator, but this has not been mentioned in lamb experiments [ 38 , 39 ] and little experience was present within the experts [F1].…”

“…One limitation of our study is that we did not insert arterial line for arterial blood sampling to avoid complications that have higher incidence of occurrence in this age group. 24 Instead, we used SpO 2 and Et-CO 2 to monitor ventilation relying on the correlation between SpO 2 and PaO 2 . 25 Nonetheless, acquiring arterial samples would have been more accurate, and we could have derived some important derivatives as OI.…”
