Restructuring Electrophysiology During the COVID-19 Pandemic: A Practical Guide From a New York City Hospital Network

Abstract: The coronavirus disease 2019 crisis is a global pandemic of a novel infectious disease with far-ranging public health implications. With regard to cardiac electrophysiology (EP) services, we discuss the "real-world" challenges and solutions that have been essential for efficient and successful (1) ramping down of standard clinical practice patterns and (2) pivoting of workflow processes to meet the demands of this pandemic. The aims of these recommendations are to outline: (1) essential practical steps to appr… Show more

“…During the current COVID-19 pandemic, several publications are providing guidance for clinicians and institutions to rebuild safe electrophysiological care [13][14][15]. A joint guidance from the Heart Rhythm Society, the American Heart Association, and a committee within the American College of Cardiology recommends consideration of same-day discharge, as clinically appropriate, to minimize patient exposure [13].…”

“…The fundamental goal is to avoid elective procedures in order to conserve precious resources like PPE and hospital beds and also minimize unnecessary exposure to COVID-19. A practical guide from a New York City Hospital Network describes an approach used to overcome these challenges due to the surge of COVID-19 activity in New York by ramping down and restructuring of the electrophysiology services ( 74 ).…”

“…In locations with limited testing availability or with concern for community spread, it may be prudent to consider at least droplet precautions for all electrophysiology procedures. N95 masks should be used by all operating physicians and a surgical mask for all patients ( 74 ). If needing intubation which is an aerosol generating procedure, it is recommended to consider performing in a negative pressure room either in the electrophysiology lab or inpatient ICU before bringing the patient to electrophysiology lab.…”

“…Hospital operations have been significantly altered resulting from the COVID-19 crisis, as previously documented. 35 To F I G U R E Paired comparison of QT Intervals in patients receiving hydroxychloroquine, azithromycin, and both. This paired comparison demonstrates no significant differences in QT, QTc, or QTf intervals between admission and the last electrocardiogram before death in patients receiving hydroxychloroquine, azithromycin, and both.…”
