Objective: to analyze the safety culture in surgical units of a teaching hospital in different periods of hospital management. Method: this is a descriptive and analytical study developed with health professionals working at a surgical center and at five surgical inpatient units in two different periods of hospital management in southern Brazil. The Hospital Survey on Patient Safety Culture questionnaire was answered by 73 professionals in period I (2014/2015) and by 158 professionals in period II (2017). The analysis was based on descriptive statistics and on inferential analysis and reliability analysis using Cronbach's alpha coefficient; the dimensions were considered strong when the percentage of positive answers was ≥75%. Results: the administrative transition had a positive influence on the “Staffing” and “Handoffs and transitions” dimensions and a negative influence on “Teamwork within units” (p<0.001), with no difference in the remaining nine dimensions. In both periods, no dimension was considered strong; there were no changes with regard to the notification of adverse events and to the overall safety assessment; and the overall reliability of the instrument was satisfactory (0.89). Conclusion: the administrative transition of hospital management had little impact on the dimensions of the organizational safety culture, and there are still challenges in the progressive development of this predictor of patient safety.

Patient safety and medical errors are among the most discussed topics in recent years. Identifying and reporting medical errors is one of the most significant steps toward the adoption of measures to increase patient safety. Pediatric nurses provide care to children, which is why they play an essential role in preventing medical errors and ensuring patient safety. This cross-sectional and descriptive study was conducted to determine the relationship between pediatrics nurse's malpractice tendencies and patient safety culture in Turkey. The study was conducted in the pediatric clinics of a university hospital and a public hospital in a metropolis in the northern region of Turkey. The study population included 142 nurses. A 'Nurse Information Form', a 'Patient Safety Culture Scale (PSCS)', and a 'Malpractice Tendency Scale in Nursing (MTSN)' were used to collect information. Data were analyzed by using SPSS 16 program, descriptive statistics, Independent Two-Sample t-test, one-way analysis of variance (ANOVA), Tukey test, Tamhane test, Mann Whitney U, Kruskal Wallis test and Pearson correlation tests. Nurses who had a master's degree, made a medical error, and observed medical errors made by another colleague had a higher mean MTSN score (p<0.05). Nurses aged 46 and older, with a master's degree, that worked in the pediatric unit as a manager had a higher total mean PSCS score (p<0.05). A positive and significant relationship was found between total mean PSCS and MTSN scores. This study found that pediatric nurses' level of malpractice tended to be low and their level of patient safety culture perceptions high, which is significant in terms of quality of patient care.

BackgroundEvery year, millions of patients suffer injuries or die due to unsafe and poor-quality healthcare. A culture of safety care is crucial to prevent risks, errors and harm that may result from medical assistance. Measurement of patient safety culture (PSC) identifies strengths and weaknesses, serving as a guide to improvement interventions; nevertheless, there is a lack of studies related to PSC in Latin America.AimTo assess the PSC in South American hospitals.MethodsA multicentre international cross-sectional study was performed between July and September 2021 by the Latin American Alliance of Health Institutions, composed of four hospitals from Argentina, Brazil, Chile and Colombia. The Hospital Survey on Patient Safety Culture (HSOPSC V.1.0) was used. Participation was voluntary. Subgroup analyses were performed to assess the difference between leadership positions and professional categories.ResultsA total of 5695 records were analysed: a 30.1% response rate (range 25%–55%). The highest percentage of positive responses was observed in items related to patient safety as the top priority (89.2%). Contrarily, the lowest percentage was observed in items regarding their mistakes/failures being recorded (23.8%). The strongest dimensions (average score ≥75%) were organisational learning, teamwork within units and management support for patient safety (82%, 79% and 78%, respectively). The dimensions ‘requiring improvement’ (average score <50%) were staffing and non-punitive responses to error (41% and 37%, respectively). All mean scores were higher in health workers with a leadership position except for the hospital handoff/transitions item. Significant differences were found by professional categories, mainly between physicians, nurses, and other professionals.ConclusionOur findings lead to a better overview of PSC in Latin America, serving as a baseline and benchmarking to facilitate the recognition of weaknesses and to guide quality improvement strategies regionally and globally. Despite South American PSC not being well-exploited, local institutions revealed a strengthened culture of safety care.
