Retroperitoneal unicentric Castleman’s disease: A case report

Abstract: HighlightsRetroperitoneal Castleman’s disease in the peripancreatic region is a rare entity.Preoperative diagnosis is most often difficult and mimics malignant disease.Surgical en bloc excision is the most commonly performed treatment due to diagnostic dilemma.Preoperative biopsy in cases with strong clinical suspicion can avoid radical surgery.

“…A study performed by The American Society of Hematology used a humanized anti IL-6 receptor antibody to treat patients with multicentric plasma cell or mixed type CD. After the administration of this antibody Weisenburger et al [16], a clinical response was achieved [11][12][13][14]. Also, when histopathological examination was performed, pathological response was noted with a reduction of follicular hyperplasia and vascularity [15][16][17][18].…”

“…For this reason, radiotherapy should be considered for a poor surgical candidate or for unresectable unicentric CD. A study by Yoshizaki et al [7] demonstrated that, in fact, the cells present in the germinal centers of the hyperplastic lymph nodes produce IL-6 and found a correlation of this production with the serum levels of IL-67 Abdessayed et al [13]. Thus, another treatment modality that has been proposed is the blockage of the IL-6 dysregulated overproduction [7,11], especially in patients with plasma-cell variant [11] & Ren et al [14].…”

“…In addition, the patient's serum levels of carcinoembryonic antigen (CEA), cancer antigen (CA) 19-9, CA125, and interleukin 6 (IL-6) were within the normal range. While the mass is usually detected incidentally, as Nakata et al (9) reported, it can also manifest itself with specific symptoms caused by the effect of the mass or adjacent organs (10,11). There have been individuals reported with palpable masses and dyspeptic symptoms (10,11).…”

“…The HP examination showed large lymphoid follicles in the form of "onion skin", atrophic germinal centers with concentric rings of B-lymphocytes. On the other hand, a hyalinization at the stromal level was found, without an aggregation of the plasma cells [47].…”

“…As a particularity of the case, we have to mention that the identification of the origin of the retroperitoneal mass was facilitated by the fact that the patient had an ipsilateral ovarian cyst [33]. Abdessayed et al (2017) published another case of a 34-year-old patient complaining of epigastric pain, especially postprandial. The results of the biochemistry examinations did not reveal anything significant.…”
