Reversibility of Lymphomas and Lymphoproliferative Lesions Developing Under Cyclosporin-Steroid Therapy

Abstract: SummaryPost-transplant lymphomas or other lymphoproliferative lesions, which were usually associated with Epstein-Barr virus infections, developed in 8, 4, 3, and 2 recipients, respectively, of cadaveric kidney, liver, heart, and heart-lung homografts. Reduction or discontinuance of immunosuppression caused regression of the lesions, often without subsequent rejection of the grafts. Chemotherapy and irradiation were not valuable. The findings may influence policies about treating other kinds of posttransplanta… Show more

“…In these 2 patients, the lymphomas spontaneously regressed after withdrawal of MTX therapy (1). The spontaneous disappearance of the lymphomatous process in these 2 patients was similar to the regression of lymphoproliferative disorders seen upon withdrawal of cyclosporine therapy in transplantation patients (2).…”

“…Patients treated with immunosuppressive agents after organ transplantation are at risk for the development of PTLD (2). The majority of PTLDs have a B cell phenotype: only rare T cell proliferations have been reported.…”

“…2 This could be ascribed to (1) a better knowledge and handling of immunosuppressive drugs, (2) a preliminary use of FK506 by Starzl et al 24 ; and (3) a population consisting of adult patients. 25 Indeed a high incidence of LPD has been reported in children treated with FK506.…”

“…8,19,20 EBV-associated PTLD appears to be most effectively treated by tapering of the doses of the immunosuppressive drugs used to prevent transplant organ rejection. 17,21 Because different viruses can give rise to similar organ pathologies, [22][23][24][25][26][27][28][29] selection of the appropriate therapeutic approach involves accurate diagnosis of disease etiology.…”
