Riboflavin/UVA Photochemical Therapy for Severe Infectious Keratitis

Abstract: The adjunctive use of riboflavin/UVA photochemical therapy has a positive effect on refractory infectious keratitis. The treatment seems to be safe and effective and should be considered as part of the first-line therapy in severe cases of infectious keratitis.

“…4 In an in vitro study, it has been shown that riboflavin/UVA was effective against some clinical bacterial isolates; 5 additionally, CCL is clinically used as a treatment option for resistant microbial keratitis. 6,7 Although there is a belief that corneal collagen cross-linking acts as an antimicrobiologic agent, microbial keratitis after CCL has been reported in some cases. 8,9 The possible explanations of corneal infection after CCL are the disruption of epithelial integrity, long exposure time during treatment, the extended or overnight wear of bandage contact lenses, and antibiotic resistance of the ocular flora, which may change after UVA.…”

“…Being retrospective studies, they are limited by the accuracy of medical records. In addition, several of the reported cases were culture negative,36 38 39 casting doubt on the accuracy of infectious aetiology. In all cases, topical antimicrobials had been used before and after the cross-linking procedure, which in the absence of a control group limits any conclusions that can be drawn from these studies.…”
