Risk Factors and Prognosis of Hemorrhagic Cystitis after Allogeneic Stem Cell Transplantation: Retrospective Analysis in a Single Institution

Abstract: 2304 Background: Hemorrhagic cystitis (HC) is a major complication after allogeneic hematopoietic stem cell transplantation (SCT), and is life-threatening in severe cases. Early onset HC is usually due to toxic effects of conditioning regimen, while late onset HC is caused by viral or bacterial infection, and GVHD. In order to find out risk factors and to analyze prognosis of late onset HC, we retrospectively reviewed data in our institution from 1996 through… Show more

“…The connection between HC and GVHD has been studied for many years, but still seems to be unclear [10,[36][37][38]. Other studies have presented GVHD as a risk factor for the BKV-HC, especially severe or late-onset HC [36,[39][40][41]. However, it still remains not clear whether GVHD targeting bladder epithelium is manifested as HC or whether immunosuppressive drugs used to treat acute GVHD as well as GVHD itself contributes to HC and its grade [42,43].…”

“…Most of our HC cases had grade 1 or 2 disease and late onset. Many risk factors have been identified for development of late-onset HC, such as older age, myeloablative conditioning, unrelated donor transplants, bone marrow as a stem cell source, delay of platelet engraftment, presence of BK virus, CMV disease and acute GVHD [18][19][20][21]. In this retrospective study, we demonstrated that CMV viraemia was an independent risk factor of HC, however, in patients with grade 2 and further HC, BK viraemia and myeloablative conditioning were found to be risk factors.…”

“…A number of retrospective studies have identified various risk factors for HC after allo-HSCT. These included busulfan (BU)-containing conditioning regimens, unrelated donor, the occurrence of graft-versus-host disease (GVHD), and CMV reactivation [6][7][8][9].…”
