Risk factors related to operative morbidity in patients undergoing gastrectomy for gastric cancer

Abstract: Morbidity of gastrectomy for gastric cancer in our institution is still high but mortality has decreased significantly over the last ten years due to the specialization of the hospital and the surgical team. The main risk factor for complications was pancreatosplenectomy in the multivariate analysis.

“…These results are consistent with our previous experience with 434 gastric cancer patients operated with nonneoadjuvant therapy (38.4% morbidity and 2.7% mortality) [14] and with those published by specialized centers in Western countries. Their results are comparable to ours regarding the incidence of anastomotic leakage (1.2-8.1%) and pancreatic fistula (0.2-5.3%), and slightly lower for abdominal abscess (1.8-4.2%), wound infection (1.1-3.9%), and postoperative intraperitoneal hemorrhage (1.2-1.9%).…”

“…Surgical mortality was defined as mortality within the first 30 days after surgery or during the same hospital stay. Analysis of complications between neoadjuvant series was also compared with our previous published experience for gastric cancer patients with nonneoadjuvant therapy [14].…”

“…2,3 Some recent Western findings are similar to those reported in Japan, assuming the operation is done at an experienced center. 4,5 In terms of minimally invasive surgery, several laparoscopic treatments for gastric cancer, such as laparoscopic wedge resection, 6 intragastric surgery, 7 and laparoscopic gastrectomy, 8 have been introduced over the past two decades. Moreover, laparoscopy-assisted gastrectomy (LAG) has been rapidly adopted in Korea and Japan for the treatment of early gastric cancer.…”

“…Due to the more extensive anatomic separation and resection compared with conventional gastrectomy surgery, the incidence of intraoperative-associated injury and post-operative complications is significantly increased (1,2). The overall morbidity and mortality rates of post-operative complications have been reported as 38.4% and 2.7%, respectively (1).…”
