Risk of Lymphoproliferative Malignancy in Relation to Small Intestinal Histopathology Among Patients With Celiac Disease

Abstract: The risk of lymphoproliferative malignancy in celiac disease is dependent on small intestinal histopathology, with no increased risk in latent celiac disease.

“…The lack of dietary compliance and persistence of histological damage, are two factors associated with the development of lymphoproliferative disease in adults, which is the most severe complication associated to CD [34] . A recent Swedish population study noted that celiac patients with persistent villous atrophy, during follow-up, had twice the risk of developing a malignant lymphoproliferative disease, mostly T lymphoma [35] .…”

“…6 Including patients with confirmed normal histopathology (but positive coeliac disease serology) in their study therefore risks underestimating the true risk of death and malignancy in coeliac disease. 7 (iii) British data suggest that internal comparisons are better than external comparisons.…”

“…[3][4][5] Our serology-only mortality analysis includes the screening studies but also includes Ludvigsson et al 6 which examined a population of serology-positive, biopsy-negative patients that were not screened. In contrast to the screening studies, Ludvigsson et al found that serology-positive, biopsy-negative patients had an increased risk of mortality.…”
