Risk-stratified lifestyle intervention to prevent type 2 diabetes

Abstract: BackgroundLifestyle intervention (LI) can successfully prevent type 2 diabetes, but response to LI strongly varies depending on risk subphenotypes. We tested if individuals with prediabetes and a high-risk phenotype benefit from an intensification of LI.Methods and findingsWe conducted a risk stratified multicenter randomized controlled intervention study over 12 months with additional 2 year follow up. In eight University Hospitals in Germany, 1105 individuals (female 59%, age 58±11 years, BMI 31.1±6.0 kg/m2 … Show more

“…Quantification of ectopic lipids, e.g. in skeletal muscle (IMCL), liver (IHL) or pancreas (PF) has gained a lot of interest in cross-sectional and longitudinal studies as ectopic lipid accumulation is a well-known crucial contributor to the pathogenesis of metabolic diseases (1,4,(8)(9)(10)22,29). Thus, non-invasive MR-based phenotyping including fat quantification is increasingly being applied in large-scaled epidemiological studies as well as in interventional prospective studies.…”

“…Due to the increasing prevalence of metabolic diseasesas type 2 diabetes mellitus and cardiovascular diseases, there is a rising interest in detailed phenotyping of persons at risk. This is done during interventions, including lifestyle modification (1)(2)(3)(4) as well as in epidemiological studies in the general population such as the Cooperative Health Research in the Augsburg Region (5), the German National Cohort (6) or the UK Biobank (7). For this purpose, magnetic resonance imaging (MRI) and proton magnetic resonance spectroscopy ( 1 H-MRS) offer established and thorough techniques for quantification of whole-body adipose tissue distribution and assessment of ectopic fat deposition in abdominal organs as liver and pancreas, and in skeletal muscle.…”

“…Samples from 52 randomly selected participants of the prediabetes lifestyle intervention study ( 38 ) (PLIS, registered with clinical trial.gov NCT01947595) were investigated in this project. None of the study participants took any kind of medication that interferes with glucose or energy metabolism.…”
