Robotic voluminous paraesophageal hernia repair: a case report and review of the literature

Abstract: Background: The treatment for sliding esophageal hernia with mild gastroesophageal reflux is usually conservative, but surgical treatment is recommended for refractory sliding esophageal hernia, paraesophageal hernia liable to prolapse, or paraesophageal hernia with ulceration and/or stenosis. Robotic surgery overcomes laparoscopic pitfalls by providing steady-state three-dimensional visualization, augmented dexterity with endo-wrist movements, and superior ergonomics for the surgeon. Case presentation: To inv… Show more

“…The most used access is laparoscopic, with better exposure of the gastroesophageal transition, facilitating myotomy, and fundoplication. 6 8 , 11 The main advantages of the laparoscopic approach compared to thoracotomy is greater safety, less postoperative pain, shorter hospital stays and systemic inflammatory response, and a faster return to work activities. However, there are still few reports of similar treatment using the robotic platform that results are expected to be close to better than laparoscopy.…”

“…Although most of the esophageal hiatal hernia has been operated by laparoscopy, and even a large hernia can be repaired by robot in the abdominal cavity [21], for the doctors who are familiar with intrathoracic hernia repair in the past, although it is difficult to perform transthoracic hernia repair with thoracoscopy in the past, it can be completed well with the help of robot. Robot assisted cardiac achalasia RAHD (robotic assisted Heller Dor procedure) operation ensures the delicacy of esophagotomy with low incidence of intraoperative and postoperative complications.…”
