Role of fundoplication in treatment of patients with symptoms of hiatal hernia

Abstract: Gastroesophageal reflux disease (GERD) is often associated with hiatal hernia (HH). However, the need for fundoplication during hiatal hernia repair (HHR) remains controversial. The objective of this study was to evaluate the effect of HHR with concomitant laparoscopic Nissen fundoplication (HHR-LNF) in HH patients. A total of 122 patients with symptomatic HH were randomized to receive either HHR (n = 61) or HHR-LNF (n = 61). The measures of evaluating outcomes included DeMeester scores (DMS), complications, R… Show more

“…The prevalence of elderly patients with giant hiatal hernia continues to rise with increasing life expectancy in an ageing population (11). Some studies consider elderly patients undergoing giant hiatal hernia surgery to be at higher risk of complications and mortality because of their advanced age and comorbidities (25,26). Gangopadhyay reported that an older patient group (≥75 years) undergoing laparoscopic paraesophageal hernia repair were associated with more complications than other groups (65-74 years group and <65 years group) (27).…”

“…The authors concluded that adding routine fundoplication is reasonable, without adding significant morbidity or side effects. The largest randomized controlled trial was conducted by Li et al (27). A total of 122 patients were randomly assigned to AR or NAR.…”

“…These results are quite similar to the more recent comparative studies included in our review. Overall postoperative GERD rates were found in 16-45% of NAR patients and in 0-19% of AR patients (19)(20)(21)(22)(23)(24)(25)(26)(27). While four of the included studies recommended selective or no fundoplication, most of these data come from earlier retrospective studies, and the presence of an important selection bias should be considered.…”
