Role of Human Papillomavirus and p16 Staining in a Patient With Head and Neck Cancer Presenting With a Synchronous Lung Nodule: A Case Report and Review of the Literature

Abstract: A 67-year-old man with a 50 pack-year smoking history, who quit smoking 30 years before, presented complaining of dysphagia, odynophagia, hemoptysis, and a 20-pound weight loss over 3 months. A positron emission tomography/computed tomography scan showed a 2-cm, [ 18 F]fluorodeoxyglucose (FDG) -avid, left upper lobe nodule, a 3-cm FDG-avid left tonsillar mass, and a 2-cm FDG-avid left level IIa lymph node. The patient underwent a transoral biopsy that revealed squamous cell carcinoma (SCC), nonkeratinizing typ… Show more

“…Because its high sensitivity for HPV, we could assert that P16ink4a a surrogate marker for HPVhr 16 in NPC and that resultis compatible with other studies like (Walline, Komarck et al, 2013, on the other hand it was not the same conclusion in studies like Bonomi and his colleagues' study [23], and our explanation is because of various reasons, like size of sample, criteria of expression and variety of methodology used in detecting both the P16ink4a marker and HPVHR 16 virus.…”

“…However, HPV status may not be evident if a patient undergoes workup for a pulmonary nodule outside the institution where he/she was treated for HNSCC, as occurred for two patients described here. Moreover, distant metastases have rarely been reported as the initial presentation of HNSCC . In such cases, other clinical variables can suggest HPV as an etiology.…”

“…Based on the clonal evolution theory, two tumors derived from a common clone suggest metastasis, while distinct clones indicate multiple primary cancers. Thus, several molecular markers, such as TP53 mutations 9 , 10 , P53/P16 immunohistochemistry 11 , HPV genotyping 12 , 13 , gene expression 14 , and loss of heterozygosity (LOH) involving microsatellite markers 15 , have been analyzed to distinguish SPT from LM in patients with double squamous cell carcinomas (SCCs). However, these molecular markers cannot provide a convincing conclusion for all patients, due to limited numbers of alterations analyzed and the existence of intratumor heterogeneity 16 .…”
