Role of imaging in diagnosis, staging and follow-up of lung cancer

Abstract: The current recommended imaging required for lung cancer staging is CT of the thorax and PET/CT from skull base to mid-thigh. However, with the recent developments in the armamentarium of imaging techniques, the choice of one of these techniques can be directed by the presence of a technique in a local hospital and/or by the presence of an experienced person at that time.

“…Early detection of lung cancer is important because the only curative therapy today is surgical resection of early-stage tumours [20]. Crucially, for patients whose lung cancers are detected at stage 1 the outcome is substantially better than average.…”

“…18 F-FDG PET/CT is widely used as imaging standard of reference for bronchial carcinoma and shows a high sensitivity (around 96 %) for the detection of malignant lung nodules. Specifi city remains lower (about 80 %), as false-positive results can occur in infectious or granulomatous disease of the lung [ 30 ]. Also in lymphoma, an entity with many 18 F-FDG-avid subtypes, 18 F-FDG PET/CT is now the recommended imaging standard of reference for staging and restaging [ 31 ].…”

“…3,5 O carcinoma escamoso apresenta lesões centrais em 65% dos casos, podendo obstruir o brônquio e evoluir com atelectasia, consolidação (pneumonia obstrutiva), cavitação (10 a 20%) e abscessos. 2,6 No adenocarcinoma encontra-se frequentemente (60% a 70% das vezes) na forma de massa periférica. A cavitação é rara.…”
