Background: Bladder urothelial carcinoma is an alarming urologic malignancy. Complex factors like modeling and local staging can affect treatment strategy. However, local staging, particularly the muscle invasion status, significantly influences decisions regarding treatment strategies. Therefore, this study aims to evaluate the novel advances of three-dimensional ultrasound imaging to assess local staging in comparison with conventional cystoscopy. Methods: Forty-three patients with painless hematuria and conventional cystoscopy findings of bladder mass underwent three-dimensional ultrasound virtual cystoscopy. All specimens from conventional cystoscopy were processed histologically. Results: Out of 43 participants, 18 (41.9%) patients proved to have invasive urothelial carcinoma by histopathology. The three-dimensional US had a sensitivity of 97.5% and a specificity of 100%, however conventional cystoscopy was accurate in only 53.5% of the studied cases. Furthermore, in the case of malignant ulcers, mural extension into both the submucosal and the muscle layers was more readily appreciated in multiplanar images. Conclusion: Three-dimensional ultrasound updates are promising for use in bladder tumor modeling and local staging; however, they can be of value in evaluating mural and extramural tumor extent and have proven accuracy.

Minimally invasive medicine has become mainstream because of its crucial clinical significance in providing a low risk of postoperative complications, limited blood loss, short postoperative recovery time, and small sizes of associated physiological tissue wounds. Endoscopic navigation systems comprise a research hot spot in medical science and technology and are an essential means to achieve precision medicine and improve surgical operation safety. As a core component in endoscopic navigation during minimally invasive surgery, endoscopes play a critical role in disease diagnosis and treatment. The development of endoscopic vision technologies has resulted in a renewed drive to further develop endoscopic navigation systems. Multiple endoscopic optical imaging modalities provide data sources for endoscopic vision technology, including white-light endoscopy, contrast-enhanced imaging and technologies involving magnified observation. Endoscopic vision is a specific application of computer vision involving the use of endoscopes that include instrument tracking, endoscopic view expansion, and suspicious lesion tracking in the application of endoscopic navigation. These techniques help surgeons or surgical robots locate instruments and lesions and expand the field of view of the endoscope. Although these technologies have been applied to various clinical and pre-clinical diagnoses and treatments, the use and combination of these advanced technologies in endoscopic navigation system for specific clinical requirements remains challenging. This review performs a broad survey of advanced endoscopic vision technologies and their application in endoscopic navigation systems. Finally, we discuss the challenges and future directions in implementing and developing endoscopic navigation systems.

Background: Bladder cancer is one of the most common tumors of the lower urinary tract. It is the seventh most common malignancy that is found to be widely spread in developed countries.Aim of Study: Is to compare pre-and post-contrast multidetector CT urography findings in detecting and characterizing urinary bladder neoplastic masses, using cystoscopy results as the gold standard for comparison.Patients and Methods: This comparative study was conducted on thirty patients, 25 men and 5 women. Over a sixmonth period, the patients were referred to the radiology department at Kasr Alainy University Hospitals. Patients with suspicious bladder mass(es) after clinical evaluation or for further characterization of an indeterminate bladder neoplastic mass lesion previously detected by other radiological investigation such as ultrasound examination were included in the study.Results: There was a non-significant (p=0.537) difference in the lesion size pre-contrast (8.76±9.1) compared to postcontrast (8.79±9.0) MDCT studies. Both measurements had the same distribution regarding the site and morphology of the lesions. Both measurements had insignificant ( p=0.101) difference in the distribution regarding extension of lesion. There was a significant (p=0.009) difference in the distribution regarding other findings of lesions (regional lymph nodes enlargement and back pressure changes).Conclusion: MDCT, pre-and post-contrast sequences, are completing each other in the detection and characterization of neoplastic bladder masses, with the conventional cystoscopy (CC) is still the gold standard for the evaluation of urinary bladder masses.
