Role of β2-Microglobulin in Postoperative Cognitive Decline

Abstract: These are the first in man data demonstrating relation between changes in cognition and β2M. The phenomenon was reversible which indicates its therapeutic potential.

“…The link between CKD and cognitive decline is widely acknowledged; however, little is known about the impact of AKI 40. Our own data suggest that periprocedural kidney insult can contribute to the deterioration of cognitive functions in TAVR patients with beta-2 microglobulin playing the key role in this kidney–brain interaction 41. For this elderly population, life extension can be at least as essential as maintaining mental independence; thus, the phenomenon is worth attention.…”

“…Currently, a reasonable patient-tailored approach focused on limiting risk factors is advised 13. We have already learnt from cardiac surgery experience that the introduction of “KDIGO bundle” can be beneficial 41. Moreover, the intervention with the supportive care “bundle” should encompass optimization of 1) volume and hemodynamic parameters, which in the case of TAVR means volume optimization before the procedure, reduction of PVR, bleeding, and vascular complications; 2) restriction of nephrotoxic drugs, mainly contrast agents; and 3) prevention of hypoglycemia.…”

“…Cognitive function 6 months after TAVI was assessed in 4 studies (n = 326). 21,23,43,45 Overall cognitive performance remained unchanged 6 months after TAVI (SMD = À0.06, 95% CI = À0.21-0.10; z = 0.73, p = .47) ( Figure 3). No significant heterogeneity (chi-square (3) = 1.6, P = .66, I 2 = 0.0%) was detected, and there were no differences between included studies assessing cognition using different scales in subgroup analysis (chi-square (1) = 0.51, P = .47).…”

“…Beta-2-microglobulin is a kind of small molecule globulin produced by lymphocytes, platelets and polymorphonuclear leukocytes [22]. It is composed of a single chain polypeptide of 99 amino acids, with molecular weight of 11,800.…”

“…It is composed of a single chain polypeptide of 99 amino acids, with molecular weight of 11,800. In patients with long-term hemodialysis, it can aggregate into amyloid fibers that deposit in joint spaces, a common complications, known as dialysis-related amyloidosis [22]. Therefore, it is significant to remove the redundant Beta-2-microglobulin during the hemodialysis.…”
